What We Do

We serve families who want to provide for family members who are disabled due to mental and /or physical disabilities.

PLAN does not replace existing service providers. PLAN works with them to ensure that the maximum level of service is provided for your family member.
Personal Plan of Care

PLAN works with family members to help develop a Personal Plan of Care. Services may include:
  • Regular personal contact.
  • Transportation
  • Liaison with community services
  • A monitor to assure benefit continutity.
  • Money management.
  • Prevention or resolution of a crisis.
  • Household and personal shopping
